Daventry and South Northants PCT and North Oxfordshire PCT Partnership are the Primary Care Trusts, which provide health services in this geographical area.

Healthcare Issues
A person’s overall health can govern their life, but it doesn’t have to. There are services available to help people living at home to have a better quality of life. Access to Rehabilitation and Care services (ARC) at North Oxfordshire PCT Partnership will arrange for an assessment in the person’s home to establish if the person requires any additional help or adaptation to their home. This could include any area of their life in the home such as:

  • Help in maintaining personal care and hygiene
  • Aids or improved techniques to improve mobility.
  • Advice or equipment to make meal preparation easier and safer
  • Advice/help in balancing their needs with their ability.

After the assessment the ARC team will refer the person to the appropriate agency or professional, such as occupation therapist, physiotherapist, district nurse, GP etc. Most rehabilitation is carried out in the person’s own home but other options include day hospitals and short in patient stays.

Falls Services at Daventry & South Northants PCT
Falls are a major cause of disability and mortality due to injury in people over the age of 65 in the UK. The aim is to reduce the number of falls, which result in serious injury and ensure effective treatment and rehabilitation for those who have fallen.

Medication Review Service at Daventry & South Northants PCT
The hazards of prescribing and taking inappropriate medicines have long been recognised. This is especially a problem in Older People. Adverse reactions, polypharmacy and non-compliance can all lead to hospital admission. Patients are most at risk after a stay in hospital when medications may have been changed. Targeted medication review can enable people to maintain their level of independence and avoid admission to hospital.

Principles of medication review -

  • Patients have a chance to raise questions about their medicines.
  • Medication review seeks to improve or optimise impact of treatment.
  • Any changes are agreed first with the patient and prescriber.
